How to get the most out of virtual primary care?

From the onset, the pandemic has acted as a powerful catalyst, accelerating the introduction of reforms and the experimentation with new models of care, many of which had long been simmering in the labs of European health systems. These transformations have quickly and deeply affected primary care due to the indispensable – and little recognized – role it has played during the pandemic.

Primary care, on many occasions in a chronic context of precariousness and lack of resources, has played a dual role: diagnosing and monitoring COVID-19 patients who did not require hospitalization (the vast majority) and maintaining the rest of the essential services, being of special importance the follow-up of patients with chronic diseases and non-delayable and urgent cases. Among these transformations, the most radical was the overnight change in the conditions of access to primary care services. In a matter of weeks, primary care went from being fundamentally face-to-face to being almost exclusively virtual with the aim to preventing infections in health centres and protecting both patients and health professionals.

Virtual primary care is here to stay

The combination of various modes of provision of primary care services is a reality. These include face-to-face visits, both in health centres and in homes or community activities; face-to-face visits by mobile teams, especially in rural settings; and virtual visits (online appointment systems, telephone consultations, videoconferences, SMS, emails). All of them constitute a fundamental feature of primary care for the present and the future.

For this reason, and after more than two years of the pandemic, it is more important than ever to analyze international experiences to ensure that virtual primary care contributes to reducing inequalities in access to primary care services and to improving its quality.

This is the intention of an outstanding report published earlier this year by the Nuffield Trust and written by Dr Charlotte Paddison and Isabelle McGill entitled "Digital primary care: Improving access for all?". The authors carried out a quick review of the national (United Kingdom) and international evidence published during the years 2020 and 2021, which yields a series of important messages, which are highlighted below.

First, patients with the least need for health care, young people and people of high socioeconomic status are the most likely to benefit from virtual access to primary care. This constitutes a new form of the "inverse care law" and can enhance health inequalities since those groups that have worse health and greater health needs (people with socio-economic vulnerability, ethnic minorities and people with cognitive or communication difficulties) can see their access negatively affected. Regarding Catalonia, a recently published study concluded that remote consultations tripled during the first three months of the pandemic. Most users were predominantly female, systematically younger, more actively employed, and with less complex pathologies. This helped mitigate, to some extent, the decline in face-to-face visits in younger age groups, but also suggests that profiles with greater clinical and social complexity benefit less from non-face-to-face visits.

Secondly, virtual primary care may lead to the replacement of some access barriers (distance, time, transport problems) by others (internet access, digital skills, device capacity). The report includes an staggering fact about the United Kingdom: almost two million people do not have access to the internet and cannot afford it, so they are automatically excluded from online care.

However, the review also shows optimistic results. In situations where digital tools improve the accessibility of primary care, such tools can lead to improvements not only in access but also in the quality of care. Choosing between different consultation modalities can benefit patients who were previously disadvantaged in face-to-face primary care in two ways: by overcoming geographic barriers to access and by promoting patient autonomy. Particularly in the field of mental health, evidence shows that remote consultations increase the contact time that these patients can have with their primary care provider, in addition to expanding the scope of programs run by specialized mental health services.

Three recommendations to overcome the digital divide

The report also points out a series of recommendations for decision-makers interested in getting the most out of digital tools in primary care, while also addressing inequalities in access.

  • Commit to the right goal: to ensure that all citizens can access primary care under equal conditions.
  • Asses the impact that the change in access conditions may have on different groups of patients and clearly, identify potential “winners and losers”.
  • Introduce virtual primary care, so that it never reduces the possibilities of access but rather contributes to increasing and personalizing them based on the individual characteristics of each patient and the dynamics of use by different groups. In this sense, studies such as the one cited above from Catalonia, which characterize the profiles that most actively use teleconsultation and other virtual care tools, are essential.

In this process, primary care professionals must play a fundamental role in three areas: understanding the needs of their population, addressing access barriers by co-designing inclusive access to clinical circuits, and ensuring that access modalities are customised appropriately.

Virtuality must be a means to reduce inequalities and increase the quality

Although not mentioned in the report, it is vitally important for policymakers to study the context in which primary care services operate, before jumping into the introduction of digital solutions. Virtual primary care should never be seen as an alternative to strengthening primary care with sufficient human resources. In addition, there is a sort of magical thinking about the time that digital tools and telematic care can save health professionals. This can end up overloading, even more, the workload of primary care professionals  that isin a state of chronic unsustainability in many European countries. In Spain, the expansion of the use of remote care not only did not decrease but rather increased the work of primary care professionals. For example, since September 2020, the increase in e-consultations has not been associated with a decrease in face-to-face consultations. This increases the total number of consultations and the workload of PC professionals by adding telematic attention to the face-to-face consultations already existing. In addition, increased accessibility often goes hand in hand with increased demand, which in turn requires increased triage and demand management efforts (and resources) to prevent delays in the attention of the most relevant problems. Finally, the digital skills of professionals must be at the centre of any virtual care development strategy.

Virtual primary care should never be an end in itself, but rather a means to a greater purpose: to reduce inequalities in access to the health system and improve the quality of service provision.

Digital transformation as a new determinant of health

The Spanish Ministry of Health has recently published the Digital Health Strategy of the National Health System. The document, closely aligned with the WHO and European Union strategy on this matter, presents three main lines of action:

  1. Development of digital health services aimed at people, organizations and processes that make up the health protection system, with an equity approach.
  2. Generalization to the maximum of the interoperability of health information.
  3. Promotion of the analysis of data related to health, its determinants and the health system.

Challenging stimuli predicting the future

I reread an article published in 2014 in the British Medical Journal, "A Glimpse into the Future: Typical day in the NHS in the year 2050", in which Hannah Wilson, a student at Imperial College, imagines a healthcare system that screens patients admitted to Singapore and New York from London, within the framework of a global hospital, and which operates on patients by remote surgeons using robots. The Euthanasia Department and the Genomics Clinic are, for her, hospital departments in 2050. She describes patients convinced of the suffering of two diseases when they know their genomic sequence and also "digitally addicted" patients in times when a day may end up in an isolation pod due to a virus raised to epidemic levels.

Primary care in our country is in a good situation regarding the use of electronic medical records. The digitization of medical records began at the end of the last century and was fully deployed in the first decade of this. Such is the progress of the clinical stations used today that they are already crying out for a technological, functional, and usability update, but changing the system is complicated and very expensive, so it's easy to expect that change will only occur when the technological version will no longer justify itself.

Changing doctors for robots is a recurring theme in science fiction, but the appearance of devices with digital assistance, such as Alexa or Siri, and the promotion of self-care through smartphones, admitted The Lancet in an editorial, now seems a plausible reality. On the other hand, Harvard Medical School Professor Warner Slack said that if a doctor can be replaced by a computer, then this doctor deserves to be replaced by a computer. In this context, Eric Topol in "Deep Medicine" brings light to this debate with the defence of an open thesis: "artificial intelligence (AI), instead of replacing doctors, should become an aid for them to recover lost humanism."

Health care between the countries of the European Union is regulated by Directive 2011/24/EU of the European Parliament and the Council, on the application of the rights of patients in cross-border healthcare, published on March 9, 2011, One of the objectives of the directive is to regulate access to patient information. The exchange of health information is important to ensuring quality care and allowing continuity and coordination beyond the borders of the different health systems existing in Europe, whether at national or regional level.

Cyberchondria: improper use of the internet increases healthcare waste

The growth of the internet has made health information more accessible than ever. Increasingly more people are obtaining health information on the internet, which has probably become the most popular method of gaining knowledge in this field. The internet has brought great benefits and opportunities, such as offering instant answers about the nature, cause, prevention and treatment of any disease, as well as recommendations to maintain healthy habits, with more and more applications in the health field. We can connect to the internet with multiple devices (tablets, smartphones), in such a way that in Spain 93% of the population currently has an internet connection. However, people who are distressed or anxious about their health can access the internet for self-diagnosis and thus allegedly to calm down. This search for content on diseases aims to alleviate anxiety, but produces precisely the opposite phenomenon, increasing stress and worry. And this is even worse if the information obtained is inaccurate, incomplete and erroneous, as it often happens.

With Amazon, there is no going back

I noticed the smile on my friends' faces when I made these comments to them. I do not have enough scientific relevance to lavish myself in informative talks, so my remarks were in opinions of the same level as those referring to what Atletico will do if it does not qualify for the Champions League. They thought it was an exaggeration and that the medicine was indeed changing a lot, but no so much.

Disruption doesn't show on the tip of the iceberg

By chance, a couple of years ago, I found some images of Gediminas Pranckevicius, a Lithuanian graphic designer. Some of these images present a (small) upper part in which he describes familiar scenes, but the rest of the image (the lower part) is very complicated. The metaphor is automatic: there is (extraordinary) life in the submerged part of the iceberg.

Disruptive changes take place in the submerged part of the iceberg that, in a short time, change the image of the visible surface. Taxi is a service that is changing in a profound way. The digital press also gives us some clues. In New York, it appears that there are ten times more alternative taxis than conventional taxis, which has caused a decrease in the income of taxi drivers (by almost 30%), a reduction in the value of licenses and a negative impact on traffic due to the large number of vehicles that circulate without passengers. It's a significant problem from many points of view, and the most impressive thing is that these profound changes have taken place in less than five years.

For a more conservative medicine

Last week I discussed the influence of machine learning on clinical practice, a dynamic loaded with chiaroscuro and, for this reason, I think it is worth echoing today the manifesto "The case for being a medical conservative" written by four doctors: John Mandrola (cardiologist), Adam Cifu (internist), Vinay Prasad (oncologist -haematologist) and Andrew Foy (cardiologist). The authors clarify that their manifesto has nothing to do with politics, but that, given the dazzle of technology and the pressure of consumerism; they are forced to embrace conservatism, a way of saying enough when nobody is willing to do it.

"Machine learning" and clinical practice

Machine learning is a branch of artificial intelligence based on the idea that systems can learn, identify patterns and make decisions with minimal human intervention, as is the case, for example, with weather forecasting. When I was little, the men of the time (then there were no women in these jobs) drew isobar lines on a blackboard where they had made their predictions based only on their experience and the four sources of data that came to them from the meteorological observatories. Now, as the forecasts are made by the models, meteorologists have enough time to comment on photos sent by viewers, in addition to raising awareness of the effects of pollution and climate change.

The rise of artificial intelligence will bring changes...

During 2017, the exhibition "David Bowie is" passed through Barcelona, which reviewed the life and all the creative facets of one of the musical geniuses of the 20th century and personally, author of some of my most beloved songs, such as Changes. One of the curiosities that visitors could discover in it was the Verbasizer, a computer program that offered random combinations of words to generate new opportunities to create phrases, as a creativity assistant. Bowie was avant-garde in many ways and he demonstrated this by risking not only with groundbreaking designs but with the incorporation of technological tools.

The excesses of image diagnostics








The practice of modern medicine is subject to much pressure from the use of diagnostic imaging technologies, many of which are truly dazzling for the eyes of clinicians and patients and, as a result, health budgets are constantly increasing. This being the case, everyone agrees that more professional debate should be generated to put the matter in its place and avoid the excesses that damage resources, irradiate people and don’t add value to clinical reasoning.

The red line of health data

The great technological giants (Google, Apple, IBM or Microsoft) are recruiting scientists and experts in medical research. Their commitment to transform medicine is decided, but John T. Wilbanks and Eric J. Topol warn of possible risks in this interesting article published in Nature. Given the difficulties that medical research has always encountered in obtaining health data, the authors believe that it may be tempting for clinical scientists to access large-scale Internet projects, with significant data collection capabilities and algorithms for analysis.

But there is a risk that Google or other smaller companies such as 23andMe, will access health data with the aim of controlling the methods used to offer certain services based on digital profiles. This is what in other sectors of the Internet is called the society of the "black box". There are ads that are only offered to certain groups of people and if these algorithms incorporate health data, the authors say that pre-existing biases in our society would increase along with inequalities.

Keep calm and leave the mobile for a while

The trade fairs tend to present the technological innovations somewhat passed by the commercial magic, which puts at risk the necessary objectivity to evaluate what they will actually contribute. This is aggravated if we consider that technology is nowadays advancing much faster than human capabilities. As technological innovation progresses exponentially, the adaptive and changing capacity of individuals and organizations is logarithmic.

In general, to take advantage of the new options that technology offers, you have to know where you want to go and what to do to get there; the problem is that we just grope. The good news, however, and the fair is a good exponent, is that so many of us are doing it that the possibilities for progress don’t stop multiplying. Remember Edison when he said he had discovered 10,000 ways of not making a light bulb.
